The direct lateral approach to the hip was first described by McFarland and Osborne in 1954, based on anatomical studies that established as functional unit the continuity of the gluteus medius, gluteus minimus and vastus lateralis performed by the thick tendinous periostium covering the greater trochanter. The approach to the hip is achieved by the anterior release of this unit, with the exposition of the articulation according to our objectives. Techniques variate in type of incision, form and/or amount of desinsertion and reinsertion of the “functional unit” and use of greater trochanter osteotomy. Describe one of the variations adopted in our hospital.
