Perilunate carpal dislocations occur due to instability in the ligaments that stabilize the carpal bones among themselves. Despite their low incidence, this injury is highly severe. The aim of this study is to report a case of injury highlighting the trauma mechanism, clinical presentation, diagnosis, treatment, and follow-up. The patient was a male with a history of trauma from a fall, diagnosed with perilunate carpal dislocation through physical examination and imaging studies, requiring surgical treatment to restore carpal anatomy. It is concluded that identifying the injury accurately and making a precise diagnosis are essential, along with selecting the correct treatment to preserve limb function and prevent future complications such as carpal instability, local fibrosis, carpal arthritis, and avascular necrosis.
