UNSTABLE PELVIC RING FRACTURE IN A PEDIATRIC POLYTRAUMA PATIENT
A REPORT WITH PROLONGED EVOLUTION

Unstable pelvic ring fractures in pediatric patients are rare and usually result from high-energy trauma, being associated with significant morbidity. We report the case of a pediatric patient involved in a truck-pedestrian accident who sustained an unstable open-book pelvic fracture with bilateral sacroiliac disruption and extensive soft tissue injury. Management was performed in a staged manner, including initial stabilization, definitive fixation, multiple surgical procedures for infection control, serial debridements, negative pressure wound therapy, and skin grafting. The patient showed favorable evolution, achieving pelvic stability, infection control, and satisfactory clinical recovery.
