The adult spinal deformity has been shown to be a frequent pathology that is associated with pain and disability. In the patient assessment, certain radiographic parameters, mainly in the sagittal plane, are correlated with the clinical status and should be taken into account for the surgical treatment decision. Fixed sagittal imbalance, like iatrogenic cases after long lumbar spinal fusion, may require complex surgeries, sometime involving osteotomy, for the deformity correction and improve the clinical outcome.
